CEAS Forms

Common forms used by undergraduate students in the College of Engineering and Applied Science. This includes the Incomplete Grade Form, Petition Form, Transfer Credit Review Form, Change of Major, Add/Drop Minor, etc.

Department Course Request Forms

If you need to enroll in an engineering course that you do not have access to, visit this site to make your request to the respective department.

Internship Credit

BME students may apply for up to six technical electives. ÌýWhen completing your application, be sure to sufficiently describe the engineering/technical nature of your internship position/duties for it to be approved for technical elective credit.

ÌýÌýCEAS Independent Study Form
Students interested in pursuing an undergraduate Independent Study opportunity through the College of Engineering & Applied Science should complete this form after having connected with an instructor to supervise their independent study project.

  • BME students may earn up to 6 independent study credits that can be used towards engineering technical electives if approved.
  • If the study is with a BME faculty member, students should select BME as the sponsoring department/program and list BMEN 4840 on the form. Ìý
